Admission & Census is the hub for all resident information collected during admission and throughout the resident's stay. With minimal data entry, Admission & Census tracks resident profile information including detailed demographics, medical professionals, related party information, resource changes, UB-92 codes, Medicare Part A Benefit Period data, and more. It also integrates diagnosis, allergy, and payer information within both the clinical and financial portions of PathLinks. Comes complete with ICD-9-CM library with code and keyword search capabilities.

Streamlined assessment and care planning process flow

The MDS/RAI (Minimum Data Set/Resident Assessment Instrument) module enables you to easily create, complete, process, validate, and electronically submit your MDS assessments according to the federal and state regulations. PathLinks allows you to effortlessly enter MDS and RAP (Resident Assessment Protocol) information into an online version of the assessment with the ability to access the MDS/RAI manual at your fingertips. MDS, PPS, and Submission Schedule Reports allow you to track and schedule the required assessments to stay compliant. Quality Measure/Quality Indicator (QM/QI) calculations and built-in logic checks help ensure assessments are error-free prior to submission.

Up-to-date and accurate data allows you to always be prepared for survey

The CMS 672/802 module enables you to create and update characteristics for current residents. You can automatically calculate Census and Conditions (672) and Resident Roster (802) characteristics from the most recent MDS responses for residents. The Census and Conditions (672) and Resident Roster (802) characteristics can also be easily reviewed and updated as the condition of the resident changes, to efficiently keep this information up-to-date.

Interdisciplinary approach to comprehensive care planning

The Care Plans application provides a comprehensive library of problems, goals, and approaches that direct and guide facility personnel in providing care to a resident. Create individualized Resident Care Plans utilizing the flexible and customizable tools within PathLinks. Problems, goals, and approaches can be added to a resident care plan from one of the supplied libraries, from facility specific custom care plans, or as free text items. Users have the ability to prioritize and group resident problems, as well as assign a new occurance of a discontinued or resolved problem. Locking and auto save features prevent loss of data.

Clear, concise documentation to help manage the clinical medical record

Clinical Documentation provides various methods for assessing, completing and managing all of your residents' clinical records. Data can easily be collected and reported for User Defined Assessments, Vital Signs, Weights, Notes, and Wound/Skin issues. Facility created or customized assessment forms ensure tracking of all necessary areas per facility policies and procedures. Global Notes enable users to create and maintain notes documenting the residents' condition by discipline.

Facilitate the order management process within your facility

Physician's Orders facilitate the order management process and validate that quality clinical care has been provided to each resident. This application supports the facility’s survey process to allow surveyors to review indications (reason for drug use), doses (the right amount is being administered), duplicative therapy, and presence of monitoring. PathLinks is a flexible system that provides pre-defined and user added order categories, First Data Bank medication library, facility specific order lists, and frequencies which can be customized for your facility's specific needs/protocols. Reporting includes Physician’s Orders lists, Order Management, Medication and Treatment Administration Records, order labels, and a Custom Report that allows for query capabilities within resident orders.

Evaluation of critical MDS data to help optimize your reimbursement

Case Mix Coach is designed to be used with files from any MDS processing software. Once an MDS file has been imported from the MDS application or retrieved from the Case Mix Coach history, it is available for review and evaluation. Case Mix Coach is an invaluable resource to help identify possible inconsistencies in MDS values and report on MDS responses that, if changed (when substantiated with clinical documentation), would result in a RUGs III classification change. This can be used as a training tool to increase knowledge of changes to MDS responses and the impact on the calculated RUGS score and reimbursment.
